Privacy Act Requests

You make a Privacy Act request to the FMC’s Privacy Act Officer in person or by mail.

In Person or By Mail:

In person requests should include the following information:

  • Provide information sufficient to identify the record being requested, e.g., the individual's own name, date of birth, place of birth, etc.;
  • Provide identification acceptable to the Secretary to verify the individual's identity, e.g., driver's license, employee identification card or medicare card;

Mailed requests should include the following information:

  • Information sufficient in the opinion of the Secretary to identify the record, e.g., the individual's own name, date of birth, place of birth, etc.;
  • A signed notarized statement to verify his or her identity.
